• Club Officer Registration Form
  • This form is to be submitted any time a club and/or organization changes officers.
  Student Organization Program Form
  • This form must be submitted for any activity request or facility reservations.

General Forms

• W-9: Payer Request for Taxpayer Identification
  • This form is required to be completed and on file from any vendor prior to any payments being issued.
• Liability Release Forms
  • This form is to be completed for every participant in a high risk event.
• Digital Signage Submissions
  • Any submission to digital signage can be made using this online form!
• Vendor Contract
  • This contract should be completed between vendors and NDNU when obtaining services for any event.
