When filling out the form, please select a purpose. Below is a description of each one. Please do not attempt to fix a facilities maintenance issue or move furniture/ items. Please do not store any hazardous items (I.E., hot plates, chemicals, candles). We must ensure that we all follow all OSHA regulations, ADA, and fire codes. If you move items around and we receive a fine during the annual fire inspection, the fine will be charged to your department. We complete work orders in the order that they are received unless there is a safety or emergency issue.
Maintenance request
Use for any plumbing, electrical, building repairs, hanging pictures, or pest problems.
Custodial Request
Use for cleaning or moving furniture or boxes. Facilities will schedule a time and date based on custodial availability unless it is a safety hazard that needs to be addressed.
Please note that our custodial team only cleans individual offices upon request. Everyone is responsible for emptying their trash and recycling if they are in a private office. There are large trash receptacles located throughout the building and outside, in which you may empty your trash. The main trash, recycling, and compost dumpsters are located behind St. Mary’s. Please put the waste in the appropriate container. If the dumpsters are full, please do not add to them.
Key Request
Use the Key Request Form to order keys. Key requests take at least 1 week before they are available for pickup. Once your key request is received, you will be sent a PDF key request form for you and your supervisor to sign. Once you have all the signatures, please send the PDF to the Director of Facilities. You will be notified when your key is ready to be picked up from SM 205 unless other arrangements have been made. Please save the PDF for future needs.
Event Request
To reserve a space that is not apart of NDNU leaseback agreement. Please select the following link: Event Request Link
Please submit a request and layout before the event no later than 2 weeks. If the request is sent later than 2 weeks
Public Safety
Building Hours and Access:
All buildings will be unlocked at 7:00 AM and locked at 10:45 PM unless scheduled for an additional time. If a building is locked and you need access, please call Public Safety at 510-852-5877. Please note that we have one security guard on site; they will get to you as fast as they can. To access the building, you are required to sign a building log with your name, date, signature, and location. If you require access to a building that is not your office, please email jroth@ndnu.edu in advance to make sure the building is open. If you have a private office, we recommend that you lock it when you are not present. NDNU is not responsible for your items.

How will the sale of the campus affect Facilities?
After the property sale is finalized, there will be some changes in the Facilities Department. This will not go into effect until after the sale is complete.
- NDNU will be leasing back St. Mary’s, Campus Center (not the Café), Namur, and Madison offices. If you need to use a space that is not a part of the leaseback, you may do so by renting it (for a cost) through Facilitron. Once we have the link, we will share it with the NDNU employees.
- NDNU Facilities will not be doing set-ups for buildings that we are not leasing back.
- NDNU Custodians will start cleaning offices daily. If you do not want NDNU Custodians to clean your office, please email jroth@ndnu.edu, and your office will be removed from the cleaning schedule.
- If you have a maintenance request, please submit it through Facilitron, and the new property manager will be contacted.
- Building hours will be changing. As will the policy for locking doors.
- Exterior doors will be unlocked at 7:00 AM.
- If you have an office in Namur, Madison, or the Welcome Center and you are the last in the building, you will be responsible for locking the front door.
- Campus Center and St. Mary’s will be locked at the end of the last class of the night.
